West Fork registration information available

Registration for West Fork students may be conducted through the school website with one walk-in date. Email notices and postcards will be sent to parents/guardians regarding registration. Registration materials will be sent to those families that request a mailing, otherwise information can be accessed on the school website at www.westforkschool.org.

The following registration items need to be filled out and returned to the school:

• Registration fees form and payment.

• Student Information Form (1 per family).

• Student Permission Form (1 per student).

• Health History and Questionnaire.

• Free/reduced lunch application and instructions (if applicable).

• Insurance waiver.

Supply lists are included on the school website as well as other forms and information.
